FREE BEHAVIORAL HEALTH QUESTIONAIRES
The Utah Department of Human Services has a HealthyMinds Utah initiative. Their site provides many free questionaires mental health professionals use. May be helpful when concerned for someone else but they don't think they are depressed, or wondering if what they are struggling with is normal. The end of the survey provides government and community resources.
Questionaires for: suicide, depression, anxiety, teen anxiety, autism, parent survey for depressed teen, ADHS, alcohol use, bipolar and mood disorder, PTSD, eating disorder, gambling, post-partum depression, pyschosis, substance abuse, etc
